TABC Certification Forms

This form is used when a seller training school needs to make a change to either location address (mailing or physical) or ownership information (if 50% or less) from what was provided on the initial application.  

If a Primary Classroom Cased seller training school houses training records at any location other than the primary location, a branch application needs to be completed and submitted for each location in which records are housed.

Application submitted to be considered for approval to offer classroom based seller training instruction and issue seller server certificates and does not qualify as an In-House Seller Server School.

Instructions for the seller training course provider to be read/given to students prior to completing the standardized final exam for the course.

Sign-in sheet utilized by classroom based seller training schools for trainee's to sign in for the session.  The location date and time of the session is located at the top of the form and each trainee who attends should print and sign their name upon arrival.

Dependent upon if an In-House primary school certificate is issued to a classroom based or internet based In-House primary certification holder, the branch application requirements would default to the type of certificate issued to the primary course of instruction; either classroom or internet based.

Application submitted to be considered for approval to offer either classroom or internet based seller training instruction and issue seller server certificates to a school sponsored or operated by a retail permittee or licensee.  An In-House training provider may only train and issue seller server certificates to individuals employed by the permittee or licensee.

Form used by seller training course providers to submit corrections to trainee data after information has already been reported to TABC.

A branch internet based seller server application is required for each domain that offers a different course of instruction from the course of instruction approved for the designated primary domain.  The domains must have common ownership.  For example, if a program is approved for an English course at the primary and wants to offer the course in another language, the alternative language course would need to be applied for utilizing an internet based branch application.

Application submitted to be considered for approval to offer internet/computer based or mobile based seller training instruction and issue seller server certificates and does not qualify as an In-House Seller Server School.
